High Risk Criteria

  1. Reasonable criteria (including the CDC criteria) for individuals at higher risk for severe illness include:
  1. Age 65 and older
  2. Reside in nursing home, long-term care facility, group home, correctional facility, dormitory, homeless or with housing insecurity
  3. Underlying medical conditions:
  1. Chronic lung disease or moderate to severe asthma
  2. Significant heart disease
  3. Immunocompromised
  4. Morbid obesity (BMI 40 and greater)
  5. Diabetes
  6. Chronic kidney disease / ESRD / Coming from a dialysis center
  7. Chronic liver disease
  8. Psychiatric or substance use disorder
